$26.294 – $31.675/hour

Job Summary
The Community Health Worker (CHW) provides community health services to clients registered with the Primary Care Outreach to Seniors Program. The CHW will work closely with other members of an interdisciplinary team to provide health education, advocacy, practical assistance and support, access to other programs and services, and assists clients and their caregivers to build support networks.
Job Specific Responsibilities
Client Support and Outreach
 Works collaboratively with members of the Primary Care Team and with colleagues from various senior serving agencies to most appropriately meet the clients’ needs.
 Maintains an active caseload and performs case management. .
 Supports clients in accessing services and acts as a client advocate where appropriate.
 Accompanies client to hospital emergency, medical appointments and clinics and other appointments as needed
 Provides practical assistance and support.
 Performs health promotion activities topics to individuals in collaboration with other members of the Senior’s Outreach Team.
 Maintains accurate and complete records of client needs and ensures record-keeping is up to date. (paper and computer data entry).
 Arranges and facilitates follow-up services in collaboration with the outreach nurses and primary care practitioners of the team.
 Assist clients and caregivers to build support networks.
 Participates in program evaluation.
 Bachelor’s degree in social sciences or equivalent combination of training and experience.
 Minimum two (2) years’ experience in a community-based setting.
 Minimum two (2) years experience working with seniors.
 Knowledge and understanding of senior issues.
 Knowledge of related community resources for seniors.
 Knowledge of health promotion principles.
 Ability to assess situations and to respond appropriate.
 Interest in a variety of health issues and willingness to learn and teach others about them.
 Valid driver’s license, insurance and use of a personally owned vehicle.
 Organizational skills including time management.
 Ability to work flexible hours based on client need.
 Ability to work collaboratively in a multi-disciplinary setting, with clients and other agencies who are involved with the care of the clients.
 Ability to handle conflicting demands.
 Sound judgement and strong problem solving skills.
 Proficient with Windows-based computer systems, in particular, email, MS Word and client management systems.
 Fluency in English and French; other languages an asset.
 Knowledge of community resources in Ottawa-Carleton.
 Experience in the health and social services field.
